Welcome: I am glad that you are taking the step of seeking help. If you have not already done so, I encourage you to learn more about me and my counseling approach.
Fee, Insurance, and Cancellation Policy: My counseling fee for New Clients is a sliding-scale ($320 to $180), and the first session is half-price. The fee is per-session, not per-person, for up to three people present in a session. I accept Cash, Check or Online Payment (via Venmo, Zelle, etc.). I do not accept credit cards or health insurance, but I can provide monthly statements that you can use to seek reimbursement from your insurance carrier. Note that using health insurance (directly or indirectly) for counseling requires that you meet the criteria for a mental health diagnosis, which then becomes a permanent part of your medical record. Please make your check out to “Transitions Counseling” (or pay online) prior to your visit in order to save time during your session. If you need to cancel your session, please give a minimum of 24 hours notice or you will be billed the full fee.
Discount Availability: I offer a limited number of income-based discounts (i.e., discounts are not guaranteed). If requesting a discount please include your Annual Household Gross Income on the form below.
Session Length, 1st Session, and Paperwork: Sessions are normally 50 minutes long. You will need to complete your New Client Paperwork onlineprior to your first session. If someone will be joining you (e.g., spouse, partner, friend) please be sure they also complete their paperwork (i.e., each person attending fills out their own, separate paperwork). If you would prefer to fill out the paperwork in-person you can let me know that in the Comments below (and you will want to plan to arrive earlier for your first session). If you are scheduling an Online Session please mention that in the Comments (I will assume you are coming for an In-Person Session unless you tell me otherwise).
Response Time and Crises/Emergencies: I respond to scheduling requests as quickly as possible (usually within a few business days). I do not provide 24-hour crisis hotline services. If you are experiencing a mental health emergency please call 911 or try the community emergency resources listed in the FAQ.
Using this Form: I recommend that you read the Guidelines and Tips for using the Availability form, and also note the Disclaimers regarding email confidentiality. If the times that you need are unavailable, please list them in the Comments and I will email you if they become available. Please remember that your session is not officially scheduled until you receive a personal (non-automated) confirmation from me, and that sessions canceled with less than 24 hours notice are charged the full fee.
I also provide a YouTube Channel where I engage with anonymous questions about life, relationships and being human. While not therapy (or a replacement for therapy) it can be therapeutic to hear me respond to questions that may overlap with some of your own concerns. And you are also welcome to send me your own anonymous questions○ if you wish.

The Availability form displays my current availability and enables you to make multiple selections from the available times and then send those to me as an email. You will first receive an automated reply acknowledging your request was successfully sent (if you do not receive the automated reply you may have entered your email address incorrectly). I will then personally reply to you as soon as I am able, confirming which time your session will be (or discussing alternatives if your requested times are no longer available). Remember that your session is not officially scheduled until you receive confirmation from me.
Guidelines: ~ Click a highlighted date on the calendar to see available times for that day. Today’s available times (if any) are displayed by default.
~ Select times that you can do by clicking on the time. Selected times will be highlighted (and are added to a List of Selected Times, below the calendar). To deselect a time click on it again (or click on the X for that time in the List of Selected Times).
~ If possible, select multiple times that you can do. This helps ensure that you get a time slot in the event that other clients request the same time.
~ “Star” (*) your most preferred time or times. If all of the times you have selected are equal in preference then you do not need to star any of them (i.e., “starring” a time gives it priority over other times you have also selected). You can also use the Comments field to further explain your preferences as necessary.
~ My preferred times on a given day are displayed in bold (in the event that some of the available times are equally appealing to you).

1st Time Visitors: ~ If this is your first time coming to the Transitions website (and especially if this is the very first page you have come to) I recommend that you visit the Home page and the FAQ to learn more about my counseling approach, my fees, how I handle insurance, etc.
~ When you are ready to contact me, please be sure to specify in the Comments field: (1) if others will be joining you (I will assume your requested session is to be for yourself only unless you tell me otherwise); and (2) if your session is to be an Online Session (I will assume you are coming for an In-Person session unless you tell me otherwise).

~ You can click on the Month heading (or swipe the Calendar left or right) to switch between months (if more than one month is available).
~ You can swipe left or right in the Available Times panel to switch between available dates.
~ You can Star (*) and Unstar times from within your list of Selected Times.
~ Clicking on the Date of a time that is in your list of Selected Times will display that Date’s available times again, including times you may not have selected (i.e., it is the same effect as clicking on that Date on the Calendar).
~ You can highlight Dates on the Calendar in blue that contain Afternoon/Evening Times (4 pm and later). This will also outline those Times in blue when they are displayed. This can help you focus on those dates and times if you can only do Afternoon/Evening times.

You have indicated that you are a New Client (i.e., this will be your 1st counseling session at Transitions) and you have requested a session time for Today. Please note the following:
~ I do not take insurance, and payment (cash, check or online) is due at the beginning of each session. Please visit the Home page and the FAQ for information about fees and other common questions.
~ I normally have clients fill out the 1st Session Paperwork online (I will send you an email with instructions). If you will not be able to complete the paperwork online prior to your requested session time please inform me (via the Comments field) and I can then leave a hard copy of the paperwork in the Waiting Room for you to fill out when you arrive (in which case I recommend that you plan to come about 10 minutes early to fill out the paperwork).
Note that these instructions are for when an individual is paying for counseling. The Fee for Organizations/Businesses is different (i.e., when an entity is paying directly for counseling or consulting).
If someone other than you (or your partner) will be paying for counseling then please use the payer’s Annual Household Gross Income. Consult the FAQ for more details.
Estimating Annual Household Gross Income: (1) On your pay stub, find the amount listed as Gross Pay or Total Earnings. Do not use the amount listed as Net Pay.
(2) Determine how often you receive payment—monthly, twice a month, every two weeks or weekly.
(3) Take your Gross Pay, and multiply it by the frequency you are paid in order to calculate your Annual Gross Pay: If you are paid once per month, multiply by 12. If you are paid twice per month, multiple by 24. If you are paid every two weeks, multiply by 26. If you are paid every week, multiply by 52.
(4) If you have a partner/spouse in your household, complete the above for them as well, and then combine the two numbers to calculate your Estimated Annual Household Gross Income.
